In the Haliburton Highlands, Pharmacists are part of the Healthcare Team at the following organizations:
HHFHT
The Family Health Team is a interdisciplinary team of health professionals, working together to provide improved access to primary care and to improve the capacity of individuals to best manage their health. The FHT is funded to provide 0.5 Full-time equivalent of a Pharmacist position. This position does not dispense medications but focuses on the following areas:
- Education of patients
- Patient case study and recommendations to family physicians and nurse practitioners
- Resource to other team members and interdisciplinary students
- Liaison to Pharmaceutical companies
- Participation in delivery of a variety of programs; anticoagulation management, smoking cessation and pulmonary education.
